Before looking at the process for the creation of a new club it is important to ascertain the 'Why?'
Moving from a club?
New clubs should mean new opportunities for players, not simply moving players from one club to another. With that in mind if you are currently based at a club it is vital that every effort is made to reconcile any differences with your current club before looking to move.
New start ups?
If you are a new club looking to set up in an area populated with clubs already- it may be advantageous to contact these clubs to see if they can host your team within their club. Being part of an established club is likely to make your team far more sustainable in the long term.
If you have come this far already you have probably given the thought of creating a new club a lot of thought, have you considered the following?
Next you need to understand the following
All clubs are required to have at least a Club Secretary, Treasurer, Chair Person and Welfare Officer. Sustainable clubs have multiple people on the committee and not one person fulfilling all roles. Please make every effort to recruit a number of people to your club, as well as trying to ensuring the committee is representative of it's members.

If you are a brand new club it is likely that you will require as a minimum, playing kit, footballs, cones/bibs etc. It is important to understand how this will be funded. Please note that there is no funding available for new clubs to be created. Funding is targeted towards current established clubs to support growth of new teams within clubs.
Funding for grassroots clubs traditionally is created in 2 ways, 1) Membership fees for players (known as subs) 2) sponsorship- grassroots clubs look to attract local businesses to sponsor playing as an example.
A County FA Staff member will be in touch within 48-72 Hours follow up with the New Club Form and create your club if suitable.
