APPLY FOR AN FA DBS
Please follow the below link and guidance to submit your application for a new FA DBS.

The first step is to select REGISTER from the login screen at the top right, enter the information requested and set your own password.
Once registered you can then login using the below organisation PIN, your email address and password created as part of the registration process..
You will be asked for information such as National Insurance No, Passport details and/or Driving Licence numbers etc so please have this information ready.
Please provide your FAN in the Personal Reference Number field provided (if known).

Once your application is submitted, the documents that you have listed in your application must be verified. You will see a list of County FA Verifiers with contact details.
All Identity Documents (ID) must be original and the FA state that the verification has to be done face to face.
How much does it cost, how do I pay
Volunteer fee: £10
Non-volunteer fee: £50
Once the application has been verified, an email with a link to payment will come directly to the applicant. Once the payment has been made by the applicant, the application can go forward to the next stage which is the police and criminal checks process.
Use of DBS Enhanced Disclosure information by The FA
By completing an online DBS Enhanced Disclosure application the applicant is allowing The FA to have sight of any criminal record or non-conviction information that is held and released by the Police via the DBS process. This information may include outstanding prosecutions and relevant allegations of criminal behaviour. In line with the DBS Code of Practice, The FA may disclose this information to those involved in making a suitability decision. The FA may use any or all of this information to help decide on your suitability to be involved with children or vulnerable adults in football.
Is it secure?
Yes. The online service is tested, approved, audited and used by several government departments. Information security management: UKAS accredited, ISO 27,001:2005.
